Flu Vaccine Report for all Licensed Facilities

Flu Vaccine Report

To comply with rule 6 CCR 1011-1 Chap 2, all licensed health care facilities must report influenza
vaccination of employees each year. The vaccination goal for 2013-2014 is that at least 75% of your
employees are immunized with the influenza vaccine.

To report these data, you can use either the Colorado Health Facility Portal (state portal) or the National
Health Safety Network (NHSN). Data need to be submitted through ONLY ONE of these systems.
Below are the parameters for both reporting systems for 2013-2014 data.

Please note that data submitted through the Health Facility (HF) Portal is due March 31.
